My MacBook connects to my iPhone hotspot but will only get an internet connection when I turn off 4g in the iPhone settings and use 3G. 4g use to work until a few weeks ago.

Try at different times of the day as 4g suffers from slow down due to congestion.

It may work fine at potentially quiet periods when less people are using 4g.

If that confirms congestion, you can use the My Network app to report the issue https://www.o2.co.uk/apps/my-network
