The policy relates to the airtime agreement as well so o2 would know that the sim that is paying the insurance was not in the phone at the time of loss / damage. I would seek advice from the insurers as per the policy documents http://static.o2.co.uk/www/docs/termsandconditions/4838_O2_Mobile_Insurance_148x210mm_16p_FA_Web.pdf

Assuming it is an o2 policy

Yes as long as the policy relates to the imei number of the handset.
I have no affiliation whatsoever with O2 or any subsidiary companies. Comments posted are entirely of my own opinion. This is not Customer Service so we are unable to help with account specific issues.

Currently using:
Pixel 7a (O2 & Lyca), One Plus 6 (Sfr), iPhone 12 Pro Max (Vodafone)

@MI5I wonder of this is dependent on who you are insured with? My policy required me to give the mobile phone number, IMEI, make and  model of the handset(s) insured. That being the case then probably the op should call their insurance just to inform them of the number change? .....

Yes definitely. I was under the impression that if the sim supplied with the phone wasn't being used in the phone at the time of any subsequent claim then you weren't covered. You do need to contact them.
