Unlatching code unsuccessful

I have been unsuccessful in unlatching my O2 locked Samsung Galaxy Note 8.

 

I filled the online form out and received an unlatching code but this did not unlock my device.

 

I then tried to fill another form out but received an email from mycare saying my request had been rejected due to multiple requests.

 

Anyone help?

Assuming you followed this process Guide: Unlocking an O2 phone to use a different SIM card ?

If so, I'll call in @Martin-O2 @Marjo or @LukasB to help.
