My wife got an email yesterday supposably from O2 telling her that her account had been compromised and click here to confirm details.

She suspected it was fake, which IMO it was so it wasnt opened and binned.

We looked at her account online and the credit was what it was supposed to be and warning messages on there.

A couple of ways around this is to Never Allow images to be Shown in E-Mails by default; when an image is downloaded from another website, this confirms that you received the e-mail.

View in Text only. Most clients will give you the option to open it up in Mail HTML if so desired.

Also look at your anti-spam settings and see if there is any advanced features such as heuristics to predict what is not actually a real e-mail such as a randomly generated domain name. Because these are being sent through real servers and masquerading their true identity, it's difficult to sort the wheat from the chaff.
