Thanks pgn, but too late. I eventually found a link online which took me straight to the exact page on O2's site that I needed. I gave up trying to find it on their site as I was just going around in circles and getting nowhere. Not an easy site to navigate, I've found.

That's why the main O2 website's "search" function was implemented @Lecky - look up at top right, click the magnifier glass and enter "PUK", which returned info as below:

Google is still miles better, if you use it as intended: Google aims to "continue to help everyone find the information that they’re looking for.".
