Financial statement request forms

Hi,

ive fallen on really hard times and my phone number has been disconnected. I called up and eventually got through to the payment support team after hours of trying and they said they will send out a financial statement request form but im not sure what this means?

is it just an income and expenditure form or does it request bank account information?

It's an affordability check so will need any income and all outgoings listed.

is this a form i fill out manually or do i send off approval for o2 to access my bank etc?

You fill it in and send it to O2.

Nothing to do with your bank but you need to be accurate with your figures as they will work out a payment plan based on the info you give them.
