If it's a PAC code you need there is a new method here (as of July 2019)

"The easiest way text PAC to 65075.

You will then receive a text message containing your PAC code along with details of any termination fees"

The only wak to get the PUK2 is to call customer service. Ask them to give you the pin2 which used to be 5555 though that may have changed now.
