Is there a way to find out on my O2 account whether I am entitled to an upgrade yet? I can't remember when I got my current phone and for how long...

As jonsie said, you can use My O2 on the O2 website.

You can also download the "My O2" app.

Connect to the internet on your phone and you can find a "My O2" section on there.

Plus you can always text "upgrade" to the number "21602".
