Does everyone on a O2 contract get the 02 refresh or is this just for new customers ?

It's all that O2 currently sell directly so any new contract or upgrade will be refresh. You may (just) be on a non refresh contract from O2 but it will be up for renewal now as Refresh has just turned 2 years old.
If you took a contract from anywhere else other than O2 it won't be refresh.

All new contracts are now Refresh but if you have two direct debits you are on an existing refresh contract.
