To unlock an iphone no code is needed it is done via itunes servers after o2 request the unlock.
To unlock an iphone once you have completed the unlock form some 3-14 days later you should get a sms saying unlocked you then put a non o2 sim in and connect to itunes that performs the unlock you should see a unlock sucessfull message. At no point do you need a code (done many iphone unlocks now).
