I have had the same problem: ipad2, O2 payg. This started happening at the very same time I got a promotion to "double my data" around June 2012. A friend who got the O2 payg ipad sim around the same time also started having this happen in June. Apparently new customers are not having this issue, which to me indicates a screwup in the old account configurations.
However, I don't have the time or patience to sit with an uninformed O2 cuatomer service person. I will switch to Vodafone.
O2 if you're listening (I would not be surprised if you're not): your failure to address this issue is a symptom of your culture. It shows you're in the decline. Again, I'm pretty sure you're not listening.

You, and others like you, seem to get some perverse pleasure in coming onto a forum and leaving one message telling everyone you're leaving - as if I care!
You also assume that moving to another network will be better. Well don't be surprised if it isn't. O2 is often rated best for customer service and although they have had a hard time during the iPhone5 launch you need to look at the bigger picture. In my opinion they have been an excellent service provider and I wouldn't change.
So good luck with Vodafone - you may need it.

Hi guys,
We’re aware of this carrier update being offered to Prepay iPad customers. If you see this offered, cancel the update as the update isn’t required.
To confirm, contract iPad customers will not see this update.
We’re working with Apple to iron out this update message. We’re sorry for any confusion this has caused.

AT LAST!! Someone from O2 finally admits that the problem exists. Gets the details wrong (contract customers DO have the problem) but, hey, it's a start.
I cancelled the SO for my contract and the problem vanished soon after so I'm happy now.

Paul I thought all ipad tariffs were payg and not pay monthly contract . http://shop.o2.co.uk/ipadtariffs/index.html
All payments are reccuring pay up front not pay after use?

Now I am in reciept of an ipad I can confirm this issue is happening and I am signed up to the 1Gb Recurring Tariff.
Just a pain each time you sync to click cancel
