Unlock sim card for iPhone 5

I woul like to unlock my iphone sim card- How can I do?

I have receive a code but don't know what to do with it.

I tried to send the O2 form but it does not work.

You need to fill in the unlock form. Not sure what code you have received but you don't get one for the iPhone,  it's done by Apple.

All you do is insert another uk non O2 SIM card to complete the unlock sequence. Such as vodaphone.

If the SIM card is blocked because you've put the sim unlock pin in too many times then it's the PUK Code you need. This is in http://www.o2.co.uk/myo2 or http://www.o2.co.uk/apps/getPUK/welcome

If your sim is blocked the phone will ask for your OUK when you turn on.
What exactly is the problem?
