That's the address but the page is now missing?
The offer is supposed to run until may 13th so not sure what's going on.

Did you check it with Sony @viridis? Was it them that told you to email it in?

No but Sony promotions are always done on sonys side.
Everytime a problem comes up, O2 say to contact Sony
Sony always keeps a full terms and conditions page with more info than O2
